Sent to Cheshire East Council less than a minute later. Council ref: 5111177.

The road surface has been degraded for at least two years and the metal grid is loosely located on its mountings. For the past nine months the noise from vehicle tyres passing over the failed surfaces has become a nuisance to us as it can be heard inside our home. The noise is constant and heard as low frequency and therefore sufficiently loud to disrupt thought.
